Nursing: Registered Nurse, Licensed Practical Nurse

 

JOB RESPONSIBILITIES
As a Home Health Nurse, you will be responsible for completing an initial assessment of patient and family to determine home care needs, developing a plan of care, and implementing that plan of care.

 

Home Health Nurse responsibilities:
•Provide a complete physical assessment and history of current and previous illness(es)
•Re-evaluate patient nursing needs and making necessary revisions as patient status and needs changes
•Use health assessment data to determine nursing diagnosis
•Initiate appropriate preventive and rehabilitative nursing procedures
•Administer medications and treatments as prescribed by the physician
•Counsel the patient and family in meeting nursing and related needs
•Provide health care instructions and education to the patient
•Identify discharge planning needs
•Act as Case Manager when assigned by Clinical Manager and assume responsibility for coordinating patient care for assigned caseload
•Participate in orientation, in-services, and staff meetings as scheduled

•Reports directly to and coordinate care with Quality Assurance supervisors

•Supervices Licensed Practical Nurse (LPN)

 

JOB REQUIREMENTS:
•Registered Nurse (RN) in the agency state of operation or Licensed Practical Nurse (LPN)
•Current CPR certification
•2-3 years of nursing experience in a hospital setting or community health setting involving direct nursing care
•Valid driver's license and insurance for the agency state of operation or surrounding area with appropriate auto insurance

 

 

 

 

Rehabilitation Therapy: Home Health Physical Therapist

 

JOB RESPONSIBILITIES
•Responsible for evaluation, planning, directing and administering physical therapy modalities of treatment as prescribed by a licensed physician.

•Administers treatments as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability.

•Assists patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.

•The staff therapist coordinates, delegates and supervises responsibilities assigned to supportive staff PTA.

• The staff therapist maintains performance improvement activities within the department and participates in QA activities.
•Ensures that patient changes are accurate and entered on a timely basis.

•Participates in all infection control, departmental equipment training, organizational safety and fire safety programs.

 

JOB REQUIREMENTS
•Current State Physical Therapy Licensure, graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy
•Current BCLS (CPR) certification within 30 days of hire
•If working/assigned to a PACE facility and Participants, CPR Certification and First Aid training required and must be maintained current throughout employment

 

 

 

 

Rehabilitation Therapy: Home Health Occupational Therapist

 

JOB RESPONSIBILITIES
•The Occupational Therapist evaluates and provides services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders.
•Occupational therapist assists clients in performing activities of all types, ranging from using a computer to caring for daily needs such as dressing, cooking, and eating.

 

JOB REQUIREMENTS
•Current State Occupational Therapy Licensure, graduate of an approved school of Occupational Therapy or graduate of accredited college or university with a certificate in Occupational Therapy.

 

 

 

 

Rehabilitation Therapy: Part Time Speech Lingual Pathologist/ Speech Therapist

 

JOB RESPONSIBILITIES

•As a Speech Language Pathologist, you will work with older adults who have trouble communicating who have lost function due to stroke, multiple sclerosis, or other illness. •You will also assist those who have difficulty swallowing.

 

JOB REQUIREMENTS
•Graduate with Master’s degree from an accredited school of Speech Therapy or equivalent Minimum one (1) year in a staff speech/language pathologist position in related work setting

•Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA

•Previous specialized home health care experience desired

JOB RESPONSIBILITIES
•Under the supervision and guidance of a licensed nurse, you will be responsible for assisting with direct patient care within the scope of your practice which supports safe patient environment.

•You will assist patient/caregiver to ensure optimal patient care and assist the healthcare team to provide and maintain a clean, safe, and attractive environment for patients. Work will include components of direct patient care, nutrition, observation, documentation, and general maintenance of the patient environment.

• You need to collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.

JOB RESPONSIBILITIES

Prepare monthly financial statements, including balance sheet, P&L, and statement of cash flows for multiple properties and business units.

•Maintain capital assets, monitor capital expenses including additions, disposal, and calculated depreciation.

•Prepare month and journal entries.

•Quarterly reconciliation of financial statements.

•Keep track of the income and expenses of the company.

•Exercise critical thinking in operational, financial, and transactional analyses to identify and help resolve any issues that may arise during daily operations and the month-end close process.

•Performs complex and technical accounting functions, including month-end and year-end closing, analyses, schedules, journal entries, and reconciliations to produce accurate financial reports; conducts expenditure analyses, identifies, and researches issues, errors, and problems and develops and provides recommendations to ensure issues are fully resolved.

•Analyze and reconcile bank statements to ensure bank reconciliations are completed accurately.

•Maintain daily cash balance and update cash forecast for executive management.

•Maintain Fixed Asset System: Manage schedules; copy invoices for audit preparation; prepare monthly general ledger journal entries for depreciation expense as well tracking all fixed assets (Computers, iPads, iPhones, etc.)

•Responsible for expense report management to as well as maintain all employee expense reports are properly submitted monthly.

•Assist Controller with Accounts Payable oversight to ensure all invoices are received and processed timely and accurately in General Ledger.

•Responsible for payroll exception reporting for management to monitor overtime, utilization of staff, and productivity.

•Prepare general ledger entries to maintain files and records and reconcile accounts.

JOB REQUIREMENTS

•Master’s Degree in Accounting or foreign degree equivalent.

•A Bachelor’s Degree in Accounting or foreign degree equivalent plus 5 years of post-baccalaureate in Accounting of which two years must be as a Senior Accountant is acceptable in lieu of a Master’s Degree in Accounting.
